You may be right, that SWI1_RULE only works for rules.
Correct me if I'm wrong:
- workitem sent to user A, as seen in workflow log
- both users A and B have received it
- you want to know how and why B has received it
If so, could you:
- show us in SWI5 that B has received it
- check in the Step History that it wasn't forwarded to B
